The surgery is done by endoscopic techiques through narrow openings or the cuts are made at sites usually shielded by clothes. One also generally use hormon blockers to hinder aspects of the puberty development to preserve childish fexibility. Also these kids are used by research institutes and medical companies for research in body modification techiques. The gymnastic schools get money for letting the kids be used this way.

I've heard it's not all bad though, since they'll earn a salary and have access to gymnast girls who are also raised to please them.
